Understanding Endometriosis

Endometriosis is a chronic condition where tissue similar to the lining of the uterus grows outside the womb, most commonly in the pelvic region. It affects an estimated 1 in 10 women of reproductive age and is a leading cause of chronic pelvic pain and infertility.

On average, it takes 7 to 9 years from the onset of symptoms to receive a diagnosis. This diagnostic delay can have a significant impact on quality of life, fertility outcomes, and disease progression.

Current European Society of Human Reproduction and Embryology (ESHRE) guidelines recommend imaging as a first-line investigation for endometriosis, rather than proceeding directly to laparoscopy. A specialist ultrasound can identify endometriomas, deep infiltrating endometriosis, and other pelvic abnormalities non-invasively, helping guide clinical management and treatment decisions.

Important Clinical Note

A normal ultrasound does not rule out endometriosis. Superficial peritoneal endometriosis and small deposits are generally not visible on ultrasound. If your symptoms persist despite a normal scan result, please discuss further investigation options with your GP or gynaecologist.

Before Your Scan

Attend with a full bladder — drink approximately 1 litre of water about 1 hour before your appointment. Finish drinking at least 20 minutes before your scan time.

During the Scan

The scan begins with a transabdominal examination (probe moved across your lower abdomen). You will then be asked to empty your bladder before the transvaginal scan, which provides more detailed views of the pelvic structures.

Our sonographers are experienced in endometriosis imaging and will proceed at a comfortable pace. You can ask questions or request to pause at any time.

After Your Scan

The sonographer will discuss initial findings with you on the day and provide relevant images.

A Consultant Radiologist will then review your scan in detail and prepare a formal report. This is sent to your GP or referring doctor within 2-3 working days.

You will also receive a digital copy of your report and images via email, which you can share with your gynaecologist or specialist.

An endometriosis screening ultrasound is a specialist pelvic scan designed to look for signs of endometriosis. It combines transabdominal and transvaginal ultrasound to assess the uterus, ovaries, and surrounding pelvic structures for endometriomas, deep infiltrating endometriosis nodules, and other markers of the condition.
Ultrasound is very effective at detecting endometriomas (chocolate cysts) and deep infiltrating endometriosis (DIE) nodules. However, superficial peritoneal endometriosis — small deposits on the surface of pelvic organs — is generally not visible on ultrasound. A normal scan does not rule out endometriosis entirely.
We systematically examine the uterus, both ovaries, the uterosacral ligaments, the rectosigmoid colon, the bladder wall, the vaginal wall, the pouch of Douglas, and the peritoneal surfaces. This thorough approach follows current international guidelines for endometriosis imaging.
